ADVENTURE ACTIVITY WAIVER & RELEASE OF LIABILITY

Please read carefully before signing. This is a legally binding agreement.

2. Acknowledgment of Risk

I understand that participation in adventure travel activities organized by Adv Travels/PSI DIVE LLC. may include, but is not limited to:

• Hiking, trekking, and mountaineering

• Cycling and bikepacking

• Scuba diving and snorkeling

• Rock climbing or technical terrain

• Water activities (rafting, kayaking, swimming)

• Remote or high-altitude travel

• Wildlife exposure

• Travel in rural or developing regions

• Use of transportation vehicles and outdoor equipment

I understand these activities involve inherent risks, including serious injury, illness, paralysis, or death.

I acknowledge that risks may arise from:

• Natural hazards

• Weather conditions

• Equipment malfunction

• Acts of third parties

• Negligence of others

I voluntarily choose to participate despite these risks.

3. Assumption of Risk

I knowingly and freely assume all risks associated with participation in these activities, whether known or unknown, foreseeable or unforeseeable.

4. Release of Liability

To the fullest extent permitted by law, I release and hold harmless Adv Travels, its owners, officers, employees, contractors, guides, affiliates, and agents from any claims, damages, or liabilities arising from my participation in any Trip or adventure activity.

This release includes claims arising from ordinary negligence.

5. Medical Acknowledgment

I certify that:

• I am physically and mentally fit to participate.

• I have disclosed any relevant medical conditions.

• I am responsible for carrying necessary medications.

• I authorize emergency medical treatment if required.

• I accept full responsibility for medical expenses incurred.

6. Equipment Responsibility

I understand that:

• I am responsible for proper use and inspection of personal equipment.

• Adventure equipment involves risk.

• Adv Travels/PSI DIVE LLC. is not responsible for personal gear failure unless caused by gross negligence.

7. Third-Party Providers

I understand that Adv Travels/PSI DIVE LLC. contracts with independent third-party suppliers and is not liable for their actions, delays, or service failures.

8. Indemnification

I agree to indemnify and hold harmless Adv Travels/PSI DIVE LLC. from any claims or legal expenses arising from my conduct or violation of this agreement.

9. Media Release

I grant permission for photographs and videos taken during the Trip to be used for promotional purposes unless I submit written notice opting out prior to departure.

10. Governing Law

This agreement shall be governed by the laws of the State of Florida. Any disputes shall be resolved in Sarasota County, Florida.

Electronic Signature

By signing below, I acknowledge that:

• I have read and fully understand this Waiver.

• I understand I am waiving substantial legal rights.

• I sign this agreement voluntarily.

• I agree that my electronic signature is legally binding.
